Direct Debit Request Explained

The CEPU Plumbing Division would like to introduce you to the option of Direct Debit. Upon payment of the current invoice enclosed, the Union would like the members to consider the new hassle free option of Direct Debit.
This is how it works:

Firstly each member is to be financial to 31/03/2007.

Fill out and return the form "Direct Debit Request (DDR)" along with your payment of current dues.

On 12/10/06 $24.00 will be deducted from your bank account marking the first direct debit. Then $24.00 will be deducted fortnightly from your account.

Should the Direct Debits form be returned after 12/10/06 a small catch up fee might have to be paid.
The direct debit amounts will accumulate against your membership, and when the next fees are due, you will be in credit and automatically sent out a clear card.
For members on payroll deductions, please note that there has been an increase of union dues and the weekly deduction amount in now $12.00. Please inform your employer of this increase.
